Middle School Only: FIELD DAY / WINTER PARTY

Winter Break is almost here. We are asking for a contribution of $25 per student. Lunch will be provided.

This year Camp Guaikinima will host a great carnival-style field adventure for our middle school kids to enjoy. The camp will provide incredible counselors entertain our students. Payments & waivers are ONLY available at @imagineptp.com/tickets from December 1st through December 15, 2019.

WAIVER: We, the parents/guardians, understand that Camp Guaikinima and Imagine Weston are organizations which makes its facilities, programs, and activities available to persons only on the condition that they agree to assume complete responsibility for any injury or damage. I acknowledge that Camp Guaikinima programs may involve risk, and assume those risks for my child. Further, in consideration, I release and hold harmless Camp Guaikinima and Imagine Charter School Weston, its officers, directors, employees and staff from any claims or damage or loss (including but not limited to physical injury and property damage) that may occur as a result of my child’s participation in any Camp Guaikinima sponsored program or activity. I hereby give the foregoing release on behalf of myself, my child, and all family members of either of us, and confirm that I am authorized to do so. I understand that Camp Guaikinima and  Imagine Charter School Weston does not carry medical/accident insurance on campers, and that I am responsible for any charges for medical treatment, property damage, or acquiring my own insurance. I acknowledge that participation in Camp Guaikinima sponsored activities is conditional upon compliance with all applicable rules and policies established by Camp Guaikinima and Sites & Adventure LLC. I further acknowledge that Camp Guaikinima and Sites & Adventure LLC sponsored activities and participants may be photographed, filmed, or videotaped from time to time, and hereby consent to use my child’s picture and likeness for Camp Guaikinima and Sites & Adventure LLC related promotional purpose without further consideration.
